Remember to email: [email protected] if your child is going to be home sick or for any reason will not be in school.
For answers to commonly asked questions, please refer to School Nurses FAQ.
For students who require medication during the school day, medication drop off will be by appointment only. Please refer to district's Medication Administration in the School Setting to print out the required forms.
It is required that I have all the following items for any medication to be accepted at school:
**Parent Authorization for Medication Administration in the School Form.
** Physician order form (you can use this school form or the one that your doctor provides. We do not require both).
In addition, students with life threatening allergies need to have:
**Emergency Health Care Plan (Allergy Action Plan)
**2 wallet size pictures of your child to be placed on the emergency Health Care Plan
- Please do not email me orders over the summer. Plan on printing everything and providing it to me when you drop off the medication.
Once you have printed and completed all the required forms please email or call me to make an appointment for drop off.
