Volunteer Forms

All school volunteers must fill out a CORI form before they are allowed to volunteer at the Eliot School. Please download the CORI Form, completing all questions and return to the Senior Administrative Assistant, Liv Nash. Please include a copy of your driver's license. Please allow two weeks before you wish to volunteer as the CORI Form must be processed by the NPS Administration Office. CORI Forms are valid for three years.
