Welcome Back to School Eliot Families!

For students who require medication during the school day, medication drop off will be by appointment only. Please refer to district's Medication Administration in the School Setting 
to print out the required forms.

You must have all the following items for any medication to be accepted:
**Parent Authorization for Medication Administration in the School Form. 
** Physician order form (can use this school template or the one that the doctor provides. We do not require both). 

In addition, students with life threatening allergies also must have:
**Emergency Health Care Plan (Allergy Action Plan)
**4 wallet size pictures of your child to be placed on the emergency Health Care Plan

Medication Drop Off Procedure 

**Medication drop off is by appointment only. Contact me to make an appointment
**Medications must be delivered to the school nurse by an adult 
**You must wear a mask and pull up to the curb (side entrance) of the school and remain in your car 
**Please have a sign with your child's last name visible in the car window 
**I will come out to your car if I do not see you please call or email me 
**All Medications must be in a pharmacy labeled container - No unlabeled medications will be accepted 
**Over the counter medications must be in the original packaging labeled with your child's name
**All over the counter medications not listed on the emergency card require an order from your doctor and parent authorization form
**All medications must be accompanied by completed Parental Authorization and Physician Authorization Forms. Forms must be dated after July 1 2020 (see link above for forms) 

COVID-19 additional requirements for attending school:

  • Please check your child's temperature daily prior to leaving home to come to school. If your child has a temperature of 100 degrees or higher they must stay home and contact the school nurse. 
  • Your child must wear a mask while at school (mask breaks will be provided). Gaiters, vented masks and bandanas will not be acceptable masks. Please have your child wear a snugly fit mask and provide them a bag to store it in when they're eating or taking a mask break. 
  • If there is a family member or close contact of your child that has tested positive for  COVID-19, keep your child home and contact the school nurse.
  • If you are awaiting the result of a COVID 19 test for your child, keep your child home and contact the school nurse.
  • Monitor for the following symptoms and if your child has them keep your child home and call the school nurse.
  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting 
  • Diarrhea
  • This list does not include all symptoms-the the CDC will continue to update this list as information develops about COVID-19. Please contact your provider for additional information.
